Patient presented to the clinic on 2/19/26 stating she received immunizations approximately two years ago. She reported that immediately after receiving her second Shingles vaccine, administered by two clinical support staff members (One was training in the MA program), she experienced significant pain. She states she was informed at the time that this reaction was a normal side effect and should resolve within a few days. She noted that she did not experience this level of pain with the first dose. The patient reported that her pain did not improve and she returned about one month later due to persistent symptoms. Over the past two years she has been referred for physical therapy, joint injections, and was most recently informed she may require surgery. She reports ongoing pain rated 10/10, inability to use her left arm, and reliance on her right hand for daily activities.
