Symptom narrative

Meningococcal Conjugate Vaccine ACWY-TT IM administered in clinic. Patient did experience a very immediate vasovagal response with vaccination and came to very quickly. Followed by pallor, lightheadedness, low BP. She reports this has never happened to her before. Has eaten today and hydrated. Had her drink water and lay down for at least 25 minutes. On final recheck BP and symptoms improved and patient was able to leave independently.
