Site: Pain at Injection Site-Medium, Site: Swelling at Injection Site-Mild, Systemic: Unable to Sleep-Medium, Additional Details: Patient presented to the pharmacy today 2/16/26 at 1215pm stating she wanted to report a reaction she had to a tdap vaccine given almost a year ago. She states since the vaccine she has been having pain at the site of injection and unable to do day to day activities without difficulties. She requested a copy of the vaccine report. I apologized to the patient, and asked if she has been seen by any md within the last 10 months for the pain she said no. She states she planned on going to ER
