Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: none to report as not able to get in touch with pt d/t address/phone-Mild, Systemic: Headache-Mild, Additional Details: Vaccine for the MMR was given IM and not SQ. It was discovered on 02-17-2026 that two powder bottles were left in the box and two prefilled diluent syringes were gone, therefore the patient was given the diluent and not the actual MMR vaccine. Tried to contact pt but out of the country, and not valid phone number, email listed but not sure if still valid email. Tech recalls the vaccine was not reconstituted. Status of pt is unknown d/t lack of ability to communicate/reach the patient.
