The nurse reported the patient is asymptomatic/No additional AE; a dose of PEDVAXHIB was mistakenly diluted with Merck Sterile Diluent and administered to a patient on 02/10/2026; a dose of PEDVAXHIB was mistakenly diluted with Merck Sterile Diluent and administered to a patient on 02/10/2026; This spontaneous report was received from a nurse and referred to a patient of unknown age and gender. The patient's pertinent medical history, concurrent conditions, drug reactions or allergies, and concomitant therapies were not reported. On 10-Feb-2026, the patient was vaccinated with a dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAX HIB) 0.5 mL, total (Lot Number: Y013471, expiration date: 06-Mar-2027) (route of administration, anatomical site and dose number in series were not reported) as prophylaxis, this vaccine was mistakenly diluted with Sterile Diluent (BAXTER STERILE DILUENT) (Lot Number: Z013233, expiration date: 07-Jul-2028) (dose and route of administration were not reported) (product preparation error, wrong product administered).
