Symptom narrative

Wrong vaccination given. Patient was supposed to receive MMR, but received varicella instead. Nurse did inform patient in the room that she was receiveing a varicella. Provider later updated nurse that patient was supposed to get MMR, not varicella. Nurse attempting to contact patient via phone to notify.

Recent PLTCS, otherwise none.

PP hypertension, hypothyroidism, HSV infection, factor x deficiency, anxiety, depression, and PTSD
