No additional AE; Nurse called reporting an AE for VARIVAX. Caller reported that on 2/13/2026, a patient was inadvertently administered VARIVAX that was reconstituted with GSK SHINGRIX DILUENT.; This spontaneous report was received from a nurse and refers to a 1-year-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 13-Feb-2026, the patient was vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), (valid lot #Z015167, expiration date: 29-Sep-2027) 0.5 mL administered by "INJ" route as prophylaxis, which was inadvertently reconstituted with sodium chloride (reported as GSK SHINGRIX DILUENT) (lot #947FS, expiration date: 18-Oct-2029) (product preparation error). No additional adverse event (AE) was reported.
