Symptom narrative

Pt received 3 vaccines: Tdap, Men A, HPV. Sat on exam table and tolerated vaccines well. Offered to lay down on exam table after their admin as pt appeared anxious, but pt declined. Pt and mother completed wellness visit and walked to front desk to check in for lab. Clinical staff heard mother screaming at front desk and pt found on floor after unwitnessed fall. Pt was rigid and not responsive to verbal commands but was conscious. Pt's rigidity improved after aprox 1.5 min. Pt AOx4 but unable to recall previous events. Vitals stable with tachycardiac. EMS contacted. Pt began vomiting without nausea. Provider contacted. Pt taken to ED for eval.
