Symptom narrative

Pt had been seen in clinic, was noted that he was due for 2nd HPV. Administered at the end of visit. Patient tolerated well. Patient and parent went to check out. While mom was checking out patient developed a red, blotchy, itchy rash on face and neck. Brought patient back to room. Full set of vitals completed. BP was elevated but other vital signs were normal. Patient with no complaints of respiratory distress. No rash or irritation at the site of vaccine. MD called to room and examined patient. Described red flag symptoms of when to be seen. Patient offered liquid oral diphenhydramine (Benadryl) but declined, will take tablets when he gets home. Pt with improving symptoms when leaving but had not fully resolved.
