Symptoms were not reported; Customer called to report TE. Vaccines were administered to patients. Symptoms were not reported; This spontaneous report was received from a medical assistant and refers to a patient of unknown age and gender. The patient's pertinent medical history, previous drug reactions or allergies and concomitant medications were not provided. On 13-Feb-2026, the patient was vaccinated with an improperly stored Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) Solution for injection (lot #Z012120 has been verified to be valid expiration reported and established as: 23-Jul-2027) which was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT) (volume, expiration date, and lot # were not reported). Vaccine suffered a temperature excursion, with a temperature -14°C up to -10°C (-13.6 C), for a time frame for 2 hours 5 minutes 0 seconds. Previous temperature excursion: no. Symptoms were not reported.
