The patient's concurrent conditions, medical history, previous drug reactions, drug allergies and concomitant therapies were not reported. On 09-Feb-2026, the patient was vaccinated with an improperly stored dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) (involved lot # was Z008873, expiration date: 14-May-2027); which was reconstituted with a sterile diluent (lot #, and expiration date were not provided); given as prophylaxis. The administered dose of the vaccine experienced a temperature excursion from -14°C up to -10°C (-13.6 C) for 2 hours 5 minutes 0 seconds. No previous temperature excursion was reported.
