Wrong vaccine administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 35-year-old female patient who received DTPa (Infanrix) (batch number 47Y5M) for prophylaxis. Co-suspect products included DTPa (Reduced antigen) (Boostrix) for prophylaxis. On 06-FEB-2026, the patient received Infanrix (left deltoid) .5 ml. The patient did not receive Boostrix. On 06-FEB-2026, an unknown time after receiving Infanrix, the patient experienced wrong vaccine administered (Verbatim: Wrong vaccine administered). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date:17-FEB-2026 The patient had come in for the T-Dap vaccine (Boostrix). The patient was unintentionally given the D-Tap vaccine (Infanrix) instead.
