the intended vaccine that was supposed to be administered was Pediarix and that, because of the similar names, there was probably some confusion; inadvertent administration of Kinrix to a patient outside the age range; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 6-month-old male patient who received DTPa-IPV (Kinrix) (batch number 42Y93, expiry date 05-JUN-2026) for prophylaxis. Co-suspect products included DTPa-HBV-IPV (Pediarix) for prophylaxis. Previously administered products included DTaP (received 2 doses on an unknown date) and Polio vaccine (received on an unknown date). On 26-AUG-2025, the patient received Kinrix. On 26-AUG-2025, an unknown time after receiving Kinrix, the patient experienced wrong vaccine administered (Verbatim: the intended vaccine that was supposed to be administered was Pediarix and that, because of the similar names, there was probably some confusion) and inappropriate age at vaccine administration (Verbatim: inadvertent administration of Kinrix to a patient outside the age range).
