Mom brought pt to the ER on 2/17/26 and then to medical facility. Pt woke up with a single red dot on her left cheek on/17/26 and it had subsequently increased to multiple dots across entire body including in hair, face, neck back abdomen chest arms and legs. In the evening 2 days prior patient was noted to use an off brand soap that mother states can often give her a rash of a similar nature. Mother denies exposure to bites. Pt came to our office on 2/20/25 for a follow up on this rash that she still had in which I called state health and senior services to discuss. After reviewing everything and photos of pt's rash they deemed this as insignificant and stated that the rash was caused by a reaction to the MMR vaccine. When pt came into our office on 2/20/26 to follow up on rash she had slight clear nasal drainage and that is all. No fever, cough, or red eyes.
Pt was seen in our office at 1/7/26 for a follow up on Salmonella. Pt was then seen in our office on 1/13/26 for issues with emesis at night.

Pt is to take Pepcid 40mg/5mL twice daily.

COVID/Flu test was done on 2-23-26 with all negative results.
