Symptom narrative

During vaccine inventory, on 2/24/26, another nurse noted the vaccine solution was not administered, only the saline syringe. I gave the vaccine the day before and did not see the vial to mix with the saline syringe. I only administered the saline to patient. I called the patient's mother as the patient is only 16 months old. The mother verbalized understanding of the error et stated she will bring child back in for her vaccine.
