Symptom narrative

Post vaccination pt presented with seizure activity. In a lot of 6 minutes pt presented 4 active seizures. Nursing staff responded to CODE BLUE, with O2 at 2L/min provided upon Provider's orders via non-rebreather mask. Emergency services activated via EMS, hemoglobin and glucose levels checked once pt stabilized. EMS arrived and proceeded to transport pt to nearest ER for further evaluation.
