Patient presented to the pharmacy requesting a flu vaccination. Technician processed the vaccination for the patient. Patient was asked if needed other vaccinations while he was in the store. Patient declined at the moment. Patient stated he had not received the 25-26 flu shot and that's all he needed today. The prescription processing did not flag or alert staff that the patient had already received the 25-26 vaccine at another Pharmacy in October 2025. Pharmacy was notified by clinical services the nest day that the patient received a second vaccination.
