On 05-Jan-2026, the patient started therapy with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate (PROQUAD)(Batch/Lot Number: Z014047, Expiration date: 03-Mar-2027) via Subcutaneous (Anatomical Site:Left Thigh) reconstituted with sterile diluent Solution for injection Asked But Unknown Batch/Lot Number via Subcutaneous. The vaccine underwent temperature excursion (-12.0 C for 40 minutes). Temperature went higher than was allowed. Nurse spoke to team as to why this could have occurred. The nurse removed built up ice in freezer.
