Patient received early 2mo vaccines (Pentacel, PCV15, and Rota) on 2/6/26 at 6 weeks of age. 4mo shots due at 10 weeks of age, on 3/6/26 or later. At patient's 2mo wcc yesterday 2/25/26, patient received Pentacel and PCV15 early. Mom noted the early dose and called our office. Mom states patient is acting well at this time with no symptoms. Discussed situation with , Dr. Advised Mom that this is an error on our part. Pentacel and PCV15 were both administered 8 days too early. In terms of safety, there is no physical harm or danger to patient in receiving this early dose. She may have normal side effects such as fever or fussiness. The timing between doses is based on effectiveness in building long term immunity, not because giving the immunizations closer together is harmful to the child's health. Unfortunately this does affect the timing of the future immunizations. Dr will call Mom when she is in the office tomorrow 2/27/26. She will review the situation with Mom, discuss a plan moving forward including measures to prevent this from occurring again and how to move forward with timing of future doses. Advised Mom to monitor for weakness, poor feeding, or any other concerns and call back if needed. Mom verbalized understanding and agreement with this plan.
