Patient was seen in office on 2/24/26 for 5 year physical, given routine vaccines- ProQuad in left arm and Quadracel in right arm. Parent called on 2/25/26 to report right arm appeared swollen, sent picture via system. Picture was sent to provider in office who recommended cool compress and Motrin. Parent called back on 2/26/26 and reported arm was red, swollen, and hot to touch, triaged by nurse who scheduled same day appointment on 2/26/26. Provider saw patient in office on 2/26/26 and reported localized reaction on posterior of right arm. Patient was given Zyrtec in office and recommended to treat at home with ice, rest, and Zyrtec daily.
