Potential for medical errors due to significant look-alike hazard between the new Shingrix prefilled syringe packaging and Havrix packaging.; the packaging similarity that could lead to confusion; This non-serious case was reported by a pharmacist via sales rep and described the occurrence of product packaging confusion in a patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Co-suspect products included HAV (Havrix) for prophylaxis. On an unknown date, the patient did not receive Shingrix and Havrix. On an unknown date, an unknown time after not receiving Shingrix and Havrix, the patient experienced product packaging confusion (Verbatim: Potential for medical errors due to significant look-alike hazard between the new Shingrix prefilled syringe packaging and Havrix packaging.) and circumstance or information capable of leading to medication error (Verbatim: the packaging similarity that could lead to confusion). It was potential for medication errors due to the colors scheme matching Havrix packaging. The actual problem with device was just potential future hazard as both the products had similar looking packaging. The batch number was not provided upon follow up with the reporter.
