Symptom narrative

1515 Vaccine given. 1555 Phone called received from patient saying that she was breaking out in hives "all over". Patient denied any respiratory issues or other problems. Instructions given to return to clinic for further observation. Pt stated she would call her mother and let me know. 1610 RN calls patient to check on her status. She stated that she went home where her mom could keep an eye on her. Instructions given by RN about OTC Benadryl and a cool bath. Reviewed signs and symptoms of anaphylaxis with instructions to go to ER if any occur. Patient stated her understanding of instructions. 2/26/26 Follow up phone call by RN. Mom stated patient was much better. Hives had faded with no further symptoms noted.
