Symptom narrative

Patient's previous vaccines administered on 9/30/25 did not transfer from her EMR to the state immunization registry. Therefore, when the doctor used the report to order vaccines needed, it showed that the patient needed vaccines that has previously been given. Patient received an extra dose of Dtap, Hepatitis B , IPV, Hiv, Prevnar 20. She had no adverse reaction, but she received injections that she did not need and these doses will need to re-administerd at the appropriate ages.
