1115- Pt looked pale and stated she was lightheaded. As nurse called for help and was assisting pt to the floor she fainted. Cool towel applied to pt.'s neck and forehead. Pt responded once placed on the floor with legs elevated. BP 138/92. Pt. was able to answer questions from nurse and then she was assisted to the sitting position on the floor. Pt. reported she had not eaten anything today. Nurse provided pt. juice and snack. After a few minutes BP 123/83. Pt was assisted to chair and continued to eat ad drink snack provided while nurses monitored pt. Pt. reported feeling OK. 1136 BP 103/75 P83. Pt. escorted to inside waiting room while next appointment was made to return. Nurse educated pt. to rest, and make sure
