Symptom narrative

Patient presented to clinic with Left leg pain from vaccines given on the 24th of Feb. Mom noticed redness and swelling in LUT on 2/26/26 around 5pm. Swelling and redness continued to grow so mom mad appt for 2/27/26 at 11:30 am. Upon exam patient had moderate amount of swelling and redness. measured length wise on leg, redness spanned 11cm and width 9.5cm. Vaccines were not given properly spaced out, as it was noticed that the spacing between the Tdap given in the LUT and the MenQuadfi given in the LLT was only 2cm or .75 inches apart from each other. Photos of spacing and redness were taken with parents' permission and documented into patient's chart. Managerial RN notified of spacing error to educate the vaccine administer ASAP. Nothing was prescribed to patient. Mom will monitor from home and update on any major changes.
