After vaccine administration patient had continued pain when lifting arms and tenderness to injection site. Physical exam by provider indicated- Upper Extremities UE distal pulses present and equal. Shoulder no deformity, tenderness, swelling, crepitus, or bruising or discoloration and Shoulder left, sensation intact, equal strength bilaterally, and Limited active range of motion; left deltoid tender to palpation on medial portion from mid delt to shoulder Palpable ridge of tissue just past midline on the medial side of delt- more pronounced when muscle is actively engaged Mild swelling of left upper arm
