Symptom narrative

Patient received Moderna COVID-19 vaccine (25 mcg, 6 months?11 years formulation). After administration, it was identified that the vaccine had been administered after the beyond-use (?Do Not Use After?) date of 02/20/2026. No adverse clinical reaction has been reported. Patient is clinically stable with no signs or symptoms following vaccination. Per CDC guidance, repeat vaccination is recommended. Parent/guardian has been notified and revaccination is being arranged.
