Intentional Age Prescription by HCP: The patient is 70 years old with no reported adverse event; Initial information received on 26-Feb-2026 regarding an unsolicited valid non-serious case received from a physician. This case involves a 70 years old and unknown gender patient who was intentionally prescribed DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a prescription of dose of 0.5ml of suspect DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE Suspension for injection (lot number, expiry date -not reported) via intramuscular route in unknown administration site for helps protect against tetanus, diphtheria, and pertussis disease (Diphtheria immunisation), (Pertussis immunisation), (Tetanus immunisation) with no reported adverse event (off label use) (unknown latency). Information on the batch number was requested corresponding to the one at time of event occurrence. It was reported "Adacel is approved for use in persons 10 through 64 years of age". Action taken was not applicable.
