Symptom narrative

nurse stated that GARDASIL 9 and PREVNAR 20 come " in the same looking big box."; no adverse event; Nurse called to report that GARDASIL9 was inadvertently administered to 6-month-old patient instead of PREVNAR20. No symptoms reported. AE was filled for that patient in case #02953553. The nurse stated that GARDASIL9 was pulled for another patient; GARDASIL9 was inadvertently administered to 6-month-old patient instead of PREVNAR20; Information has been received from Business Partner/CRO on 24-Feb-2026, and 24-Feb-2026. This spontaneous report was received from a other health professional and refers to a 6-month-old patient of unknown gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On an unknown date, the patient started therapy with Human Papillomavirus 9-valent Vaccine, Recombinant (lot #Z008509, expiration date: 23-Jul-2027), dose number 1, for an unknown indication (Inappropriate age of vaccine administered). On 13-Feb-2026, the patient experienced GARDASIL9 was inadvertently administered to 6-month-old patient instead of PREVNAR20 (Wrong vaccine administered). On an unknown date, the patient experienced nurse stated that GARDASIL 9 and PREVNAR 20 come " in the same looking big box."(Product packaging confusion). On an unknown date, the patient experienced no adverse event.
