Site: Bruising at Injection Site-Mild, Systemic: patient did not receive anything but the diluent, rph failed to reconstitute-Mild, Additional Details: Priorx was not mixed, pt only received diluent as im injection. Readministered reconstituted vaccine on 2/22/26
