Site: Pain at Injection Site-Mild, Systemic: No symptoms-Mild, Additional Details: Pts father specified MMR vaccine and tdap booster for pts school requirements, pt filled out form with MMR and tdap. MMR was given IM and pts mom clarified via paper from school that it was supposed to be varicella, pt and family was notified of the mistake and no allergic reaction occurred as they waited 15 minutes after vaccine was given.
