Symptom narrative

Site: Bruising at Injection Site-Medium, Site: Itching at Injection Site-Medium, Site: Pain at Injection Site-Medium, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Systemic: Body Aches Generalized-Mild, Systemic: Chills-Mild, Systemic: Fever-Mild, Systemic: stiffness of the neck-Mild, Systemic: Nausea-Mild, Systemic: Vomiting-Mild, Additional Details: Mother explained that pain/ fever were being managed by tylenol+ibuprofen combo, mild relief. En route to urgent care for further evaluation. Also described the site of injection as "Black, blue and red." She requested that daughter's vaccination info be emailed to her, to which I immediately obliged. Offered to address any concerns she may have as they arise.
