Per 02/09/26 Office Visit Notes with PCP [name withheld], PA-C - The patient is a 62-year-old male presenting with left shoulder pain following influenza and shingles vaccines in October 2025. He reports persistent left shoulder pain, which initially started as soreness and has now progressed to a consistent ache pain. The pain disrupts his sleep, although he retains full mobility. Patient also describes a grinding sensation in his arm/shoulder. He suspects that the pain is a reaction to the vaccines and has been managing it with Tylenol for the past three months. Additionally, he reports experiencing numbness, tingling, swelling, and pain in his hands and feet, describing the sensation as if he is constantly wearing shoes. He has a history of carpal tunnel syndrome and excessive alcohol consumption, although he has been abstinent for the past eight years. Patient was advised by PCP to continue movement to prevent frozen shoulder. He was referred to orthopedics for evaluation and possible steroid injection. Labs were also ordered to assess for cause of numbness/tingling. Per further chart review - Patient was seen and evaluated by orthopedics on 02/11/2026 - Bilateral shoulder x-rays were unremarkable, betamethasone [steroid] injection given in left shoulder and home exercise program was recommended with plan for follow up in a couple of weeks. Of Note - Patient also reported pain in left arm at 12/16/25 Visit to ER but this was not the primary reason for visit. Full details from this ER visit not available for review.

Lab tests done 02/19/2026 were unremarkable. Vitamin B12 level was a little low and supplement was recommended by PCP.
