a 2yr old patient was inadvertently administered an adult dose of VAQTA; a 2yr old patient was inadvertently administered an adult dose of VAQTA; No additional AE; This spontaneous report was received from other health professional, concerning a 2-year-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 27-Feb-2026, the patient was vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) (lot #Z006542 has been verified to be valid, expiration date reported and confirmed as 20-Oct-2026), as prophylaxis (dose and route not provided). The vaccine used was an adult vaccine (Product administered to patient of inappropriate age) (Overdose). No additional adverse event reported (No adverse event).
