Provider ordered B-12 injection for the patient. The Medical Assistant removed TDAP (Boostrix) from the refrigerator thinking it was the B-12 and proceeded to give it to the patient. It wasn't until another patient arrived for a B-12 injection and a coworker mentioned that B-12 is not refrigerated that she realized her error. She immediately reported the incident to management. The provider and patient were notified. The patient reported no adverse side effects and the TDAP vaccination was documented in the chart. The Medical Assistant is receiving additional education and we are implementing extra steps to ensure patients are receiving correct medications and vaccines.
