Symptom narrative

Patient was supposed to receive MMR and Varicella in separate syringes, however the MA that was preparing the vaccines took the wrong one out of the freezer. She administered ProQuad as well as Varivax, so patient ended up receiving two doses of Varicella. The patient's family was called and the provider's instructions passed on to the patient. Instructions were to monitor for fever and treat with Tylenol as needed. Per the provider, the patient would not need to return to the clinic or undergo any further treatment.
