No additional AE; Nurse practitioner calling to report that a dose of PROQUAD was accidently given to a 55-year-old female. The nurse was supposed to give an MMRII vaccine. No additional AE, no PQC. No additional information.; This spontaneous report was received from a nurse practitioner (also reported as nurse prescriber) and referred to a 55-year-old female patient. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 04-Feb-2026, the patient was accidently vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD)(1 dose, frequency: total, lot #Z013988, expiration date: 02-Mar-2027, strength and route were unknown) for prophylaxis, the vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT) (lot # and expiration date were unknown). The patient was supposed to be vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine (MMR II) which was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT) (dose, frequency, strength, route, lot# and expiration date were unknown) (Wrong vaccine administered).
