The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 24-Feb-2026, the patient was inadvertently administered with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate and solvent for solution for injection (PROQUAD), 0.50 mL, total, Batch/Lot Number: Y013584, Expiration date: 03-Feb-2026, for the indication of prophylaxis (route and anatomical site were not reported) after the expiry date (Expired product administered). The vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (lot# not reported, Expiration date: 28-Nov-2026). No temperature excursions (TEs) on the Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilisate and solvent for solution for injection (PROQUAD).
