Symptom narrative

Pt. came to health dept. for MMR and Varicella vaccines today for college. Pt. stated that he had vagel response after injection two years ago in the ER. Pt. started feeling dizzy and c/o ears buzzing and became diaphoretic and pale after 2nd injection today, had pt. lie in supine position on exam table until dizzyness subsided and applied ice to back of neck. Pt. feeling better after 5-10 min, pt. moved to chair and sat an additional 10 min until color pink and no further c/o dizzyness. Instructed to notify future medical staff of reactions to needles.
