Menveo vaccine given appropriately to child with functional asplenia/ Sickle Cell Disease per CDC/ AAP recommendation . Menveo10+ formulation given to child in error. Menveo10+ was available and despite researching dosing recommendation, at CDC immunizartion website and review MENVEO immunization box insert, ordering physician was not aware that there was a formula difference for children >10 yrs and <10 yrs . Patient did not have adverse reaction and no harm was done to patient. Parent advised of error. Second dose will be administered due to high risk (functional asplenia) but will use correct form for children under age 10 in 8 weeks.
