She reports that a patient was given both vaccines.; This non-serious spontaneous safety report, with reference number 2026SA014950 (Inquiry Number: 02780849), was received by partner on 15-JAN-2026 and forwarded to agency on 16-JAN-2026 from an HCP. Due to the nature of the event, the causality between the event 'Medical Assistant is trying to see any counter indication for the DAPTACEL and VAXELIS.
