Symptom narrative

10:15 am after vaccination, patient passed out. He was pale, diaphoretic and slumped in his chair. After being aroused pt came to and was able to answer questions. He was nauseated and dizzy at that time. Legs were elevated, ice pack applied to back of neck. At 10:28 Temp was 98.1; BP left arm 87/54-67. Pt denied eating breakfast. Pt was given small sips from a juice box and continued monitoring was done. At 10:40 repeat vs 86/59-64. Pt's color was now pink. Pt denied dizziness or nausea. Pt was able to walk with assistance approx 20 feet. Pt stated he felt fine. Pt was discharged in moms care with instructions to f/u with pcp. Call 911 with worsening signs/symptoms. Mom verbalized understanding.
