Focus: Adverse Reaction to Hepatitis B Vaccine (Dose 4) Data (D): At approximately 0840, 1 minutes post-vaccination, employee (21-year-old female) quietly stated she felt warm. Observed marked pallor and diaphoresis as employee lost consciousness and slumped in vaccination chair. Brief (approx. 8-10 seconds) rhythmic tonic-clonic movements observed in both arms. Pulse felt slow and thready; respirations remained regular. No tongue biting or incontinence noted. Action (A): Patient immediately placed in supine position with legs elevated. HR 46. Provided verbal reassurance as patient regained consciousness at approximately 0842. Response (R): Patient fully alert and oriented (A&O x4) by 08:45. Denied confusion or "foggy" feeling . Reported feeling warm, nauseated, and tired but okay. 08:53: HR 68. Given basin, damp cloth, dimmed all lights, and small bottle of water. Remained in clinic for 20 minutes of additional observation without further incidence.
