Symptom narrative

02/28/2026 per patient hives appeared at site of injection. Observed hive 3/3/2026 at health department and it was approximately 4-5 cm by 2-3 cm round, raised and erythematous. Advised patient to follow up with their primary care physician, patient voiced understanding.
