Potential administration of only diluent portion; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of potential for medication error in a patient who received Herpes zoster (Shingrix) (batch number 2SK93) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced potential for medication error (Verbatim: Potential administration of only diluent portion). The outcome of the potential for medication error was not applicable. Additional Information: GSK Receipt Date: 23-FEB-2026 The pharmacist mentioned that they were checking on a potential administration error, in terms that they thought just diluent of Shingrix was given and not vaccine, but they had no data on date of administration, which patient could have that administration error, they did not know if it happened, it was during last month. They had 2 lot numbers of Shingrix, one of them they were for sure it was for the diluent NY535, and second lot number was 2SK93, but that one they did not know if it was for diluent or powder. They were still investigating the situation, no further data so far.
