Symptom narrative

Patient got pail and felt like he was going to pass out after HPV vaccine was given. He was already sitting so mother told him to lay down and got attention of MA staff. He was given juice and gram crackers and provider was brought to his room. He was asked if he ate prior to vaccine and he said not eat lunch on 03/03 and "forgot to tell mom" He was given a second juice in office and was walked to car with mother and MA staff.
