Symptom narrative

Mom called facility on 3/3/26 to report that child had an allergic reaction to her vaccines yesterday. They went to clinic straight after leaving the facility for routine appointment. Mom states that by the time they arrived at the clinic facility, child had developed skin blisters, burning, and itching all over her body. She reports that clinic treated her with steroids and benadryl at her visit. Mom states that the child now has a sore throat and believes she had some swelling in her throat during the reaction. Mom requested these allergies be noted in her EHR at facility.
