RN gave patient a 3rd dose. Pt initially gave RN her wrong birthday, therefore there were no recorded of MMR ever given. Patient also declines ever receiving MMR vaccines. Later into visit patient states her birthday is actually a different day. Patient correct birthday record showed she had received 2 prior doses.
