On 03/03/2026 at 2:01 PM, patient arrived for immunizations and was screened by RN with Clinic Supervisor interpreting. The patient denied receiving vaccines in the past four weeks, and state immunization system showed no records since 2018. A flu vaccine was administered in the right arm at 2:07 PM. When preparing to vaccinate the left arm with other vaccines, a bandage was noted. The patient then reported receiving something earlier that day at a doctor's office, could not recall what was administered. Per Clinic Supervisor direction, the MD's office was contacted and confirmed that they administered a flu vaccine that morning. Client had no documentation of vaccine (no VIS or vaccine card). At 2:24 PM, Clinic Supervisor consulted the on-call physician, Dr. [withheld name], who advised that remaining vaccines could proceed and that receiving two flu doses was not contraindicated. The patient was informed, verbalized understanding, and repeated the education provided. The patient was asymptomatic and stable leaving the clinic. Education was given on the importance of disclosing prior vaccinations and asking for documentation when receiving vaccines or other medical treatments. The PHN Manager was notified of the incident at 3:00 PM.
