Symptom narrative

02/11-Both vaccines given (Pediarix in Right-impacted arm), HiB in Left. 02/12-Patient reported redness swelling beginning-sometimes common for small amount, not concerned. 02/13-concern began rising as redness swelling spread throughout upper right arm. Traveled to (name withheld) Pharmacy (where vaccine was given) after not being able to get into PCP. We were able to have on-site provider in (name withheld) Center examine patient quickly. Patient was given cephalexin (as precaution), and reaction was outlined to determine if worsening. Provided instructions on what to do/who to contact if arm worsening. 02/16-both I (pharmacist who administered), and the provider who saw patient in clinic followed up-patient drastically improved over the weekend to point of non-concern.

Valacyclovir, cholecalciferol, Ascorbic Acid, Ezetimibe, Famotidine, Other vaccine given in opposite arm same day (ActHiB).

Miralax, Esomeprazole, Pantoprazole, White Petrolatum, Latex, natural Rubber
